  • Conflict of Interest Disclosure COI Form (Your first slide in your presentation must also disclose any conflicts.  If you have no conflicts, this must also be disclosed on the first slide in your presentation)

  • A cell phone number through which we could reach you if necessary.

  • PowerPoint Presentation – Please let us know if you need any special audio-visual equipment. Handouts – In addition to your PowerPoint presentation, other handouts are encouraged(Your handouts and presentation will be available to attendees as a PDF file through the COA’s webpage as an archive of the meeting. The session will also be audio-recorded and used for future COA educational activities.)
